Like you have a BIG problem you stuck in, like in Pinocchio, they are "In the belly of a whale", they are underwater, and trapped inside a sea mammal, that is a big problem, or in the Decemberists' Mariner's Revenge song, it is literal, but it is also metaphorical, in his case, he is trapped in the belly of a whale, but he also is homeless, and an orphan.
